What are the expected program outcomes for the participants?
Expected service outcomes will be generated based upon a self-directed services model and the individualized written plan developed around the individual's goals. Participants, family members and supporters will be central to the planning process where personal choice drives the goals and ultimately, service delivery. Training options will be selected based upon the self-directed choice of individual as well as assessment results, desired outcomes and adherence to "Best Practices." The primary focus for services is employment, community access and self-sufficiency.
How is the service plan developed?
Service plan development will be person-centered and directed by the individual and his/her support team.
What is the role of outside consultants?
Outside consultants will be utilized in the identification, implementation, supervision and oversight of therapeutic interventions.
What is the role of outside family members and other supporters?
It is important for all supporters to be involved in the service development planning and implementation. Outside support for therapeutic interventions increase the likelihood of successful outcomes.
How is individual progress evaluated and how do you keep the support teams informed?
The individual's progress is evaluated on a monthly basis. Immediate access to progress notes is articulated through the use of technology. Quarterly and annual reports will summarize progress.
What is the anticipated length of time participants will be in the program or is it possibly forever?
Length of services will be dependent upon the goals that are set by each individual. We anticipate that support needs will taper as the individual develops skills and personal competencies. We hypothesize there will be a reduction in services with successful achievement of goals and objectives.
What does the assessment involve?
The initial assessments tools, instruments, and resources utilized will be unique to each individual; however, standardized testing instruments, the Supports Intensity Scale (SIS), self-reports, checklists, observations, behavior scales, job samples, productivity studies, situational assessments and computer assessments are available to all participants.
